There are several ways to purchase NEXPLANON and you can always call 1.877.467.5266 to place an order or request assistance.

Purchase via line of credit: 90-day credit terms available.

ORDERING FROM CURASCRIPT

• Complete the one-time new account application form. To place an order, call 1.877.467.5266. CuraScript also has a dedicated team to support and assist you with online orders for NEXPLANON.

ORDERING FROM CVS CAREMARK/THERACOM

• Line of credit is available. No separate account application form necessary. To place an order, simply FAX the service request form (complete the Purchase NEXPLANON section) or call 1.877.467.5266. With CVS Caremark/TheraCom you can also verify benefits and order NEXPLANON online.

Purchase via credit card

• Provide credit card information when placing your order to either CuraScript or CVS Caremark/TheraCom.

Purchase via prescription orders

• When NEXPLANON is covered as a pharmacy benefit, prescription orders may be placed for individual patients. Send the prescription using the CuraScript Benefit Verification Form or the CVS Caremark/TheraCom Service Request Form (Prescription Information Section).

When you are ready to order NEXPLANON

Orders received by 7:00 pm Eastern Standard Time (EST) will be shipped second-day delivery.

Overnight shipping is available.
